Yes. The super fertility may cause this on it’s own, but there is only one way to make sure your sim has twins. Unfortunately you cannot ensure that your sim has twins without a cheat. Seeing as you don’t want to use a cheat, this won’t be of much help, but if you’re willing to, follow these instructions. Open the cheat window (Ctrl + Shift + C) and enter boolProp testingCheatsEnabled true. Then, press shift and click on your pregnant sim at the same time. Then select “Force Twins” on the menu that comes up.
In my experience, if both adults have the fertility bonus, you get twins about 50% of the time, but that may just be me. Eating cheesecake will increase the chance of twins. If one of the parents is a twin, that can increase the chance of twins being born.
The correct cheat (in case you get desperate) is
forcetwins
It must be entered when the pregnant woman is the active sim.
